At Cats in the City, we recognize that some cats have complex medical conditions, advanced age, terminal diagnoses, or health concerns that may require important medical decisions while they are in our care.

The Advanced Medical Boarding Directive allows guardians to document their wishes in advance so our team can respond appropriately and efficiently if a medical emergency occurs during a boarding stay.

This directive helps ensure that your preferences are clearly communicated regarding emergency treatment, hospitalization, cardiopulmonary resuscitation (CPR), pain management, hospice care, euthanasia authorization, and aftercare arrangements.

Why We Use an Advanced Medical Directive

Medical emergencies are uncommon, but when they occur, decisions may need to be made quickly.

By completing this form before your cat's stay, you can:

  • Clarify your medical wishes in advance

  • Reduce delays during emergencies

  • Provide guidance if you are temporarily unreachable

  • Help veterinary teams understand your preferences

  • Support compassionate, informed decision-making during stressful situations

The directive serves as an important communication tool between you, our boarding team, and any emergency veterinary providers involved in your cat's care.

Who Should Complete This Form?

We strongly recommend completing the Advanced Medical Boarding Directive if your cat:

  • Has a chronic medical condition

  • Has diabetes, kidney disease, heart disease, cancer, or other significant diagnoses

  • Is considered a senior or geriatric cat

  • Is receiving palliative or hospice care

  • Has a history of emergency medical events

  • Has specific treatment limitations or end-of-life preferences

Guardians of healthy cats may also choose to complete the directive if they wish to document emergency decision-making preferences in advance.
