This document allows you to communicate your wishes regarding your cat's care should you become unable to provide care due to illness, injury, hospitalization, incapacity, relocation to a care facility, or death.
Cats in the City Rescue specializes in structured rehoming and individualized placement planning designed to preserve feline welfare and reduce the risk of future displacement.

In the event that I become unable to provide care due to death, incapacitation, hospitalization, placement in a care facility, or other significant life circumstance, it is my expressed wish that Cats in the City Rescue be contacted regarding the care, evaluation, and placement of my cat through its structured rehoming process.
I authorize my family members, emergency contacts, caregivers, healthcare representatives, estate representatives, veterinarians, and other appropriate parties to communicate with Cats in the City Rescue regarding my cat's welfare and transition planning.
I understand that this document serves as a statement of intent and personal wishes and does not constitute a legal will, trust, contract, or estate-planning instrument.
