This document allows you to communicate your wishes regarding your cat's care should you become unable to provide care due to illness, injury, hospitalization, incapacity, relocation to a care facility, or death.

Cats in the City Rescue specializes in structured rehoming and individualized placement planning designed to preserve feline welfare and reduce the risk of future displacement.

I understand that this document serves as a statement of intent and personal wishes and does not constitute a legal will, trust, contract, or estate-planning instrument.
