Planning ahead ensures that your cat will always have care, safety, and advocacy — even if something unexpected happens.

 

Use this checklist to confirm that the key pieces of your cat’s future care plan are in place.


1. Lifetime Care Program Enrollment  

 

☐ I have enrolled my cat in the Cats in the City Lifetime Care Program

 

☐ My contact information is current

 

☐ My emergency contacts are listed

 

☐ My cat’s care profile has been completed


2. Durable Pet Care Power of Attorney  

 

☐ I have signed a Durable Pet Care Power of Attorney

 

☐ A copy is stored with my important documents

 

☐ My emergency contact knows where this document is located

 

☐ Cats in the City has a copy on file


3. Pet Care Planning Worksheet  

 

☐ My cat’s routines and personality notes are documented

4. Emergency Notification Plan
