FELINE TRANSITIONS®
To describe the early outcomes of the FELINE TRANSITIONS® structured rehoming model and evaluate placement durability, euthanasia incidence, and rehabilitation outcomes among a cohort of medically, behaviorally, and environmentally complex feline surrender cases.
A retrospective review was conducted of all cats accepted into the Cats in the City Rescue FELINE TRANSITIONS® program between January 2025 and May 2026. Cases were assessed using a structured transition framework emphasizing veterinary review, behavioral assessment, environmental evaluation, stabilization, rehabilitation, and compatibility-based placement.
Primary outcome measures included:
adoption return rate
euthanasia incidence
placement stability
rehabilitation completion
Twenty-three cats completed the FELINE TRANSITIONS® structured rehoming process.
Outcomes included:
28/28 successful transitions
0 adoption returns
0 euthanasia events attributable to behavioral or placement failure
0 failed placements
One cat was humanely euthanized following diagnosis of advanced abdominal cancer after intake. This outcome was determined to be unrelated to rehabilitation or placement failure and represented a quality-of-life intervention following veterinary evaluation.
Early results suggest that a stabilization-first, compatibility-focused rehoming model may contribute to durable placement outcomes among medically and behaviorally complex cats. Further study is warranted.
Animal welfare systems are frequently evaluated using metrics such as intake volume, live release rates, length of stay, and adoption counts.
Less frequently examined are the durability of placements and the outcomes of cats presenting with overlapping medical, behavioral, environmental, and relational complexities.
Cats in the City Rescue was established in January 2025 to address a growing population of cats whose needs appeared poorly served by traditional surrender pathways. These cases frequently involved:
chronic medical conditions
behavioral dysregulation
environmental incompatibility
caregiver illness
household destabilization
failed placement histories
euthanasia risk
The FELINE TRANSITIONS® model was developed to provide structured assessment, stabilization, rehabilitation, and compatibility-based placement prior to adoption.
This paper reports the initial outcomes of that model.
The FELINE TRANSITIONS® model utilizes a structured assessment process prior to acceptance.
Cases undergo review of:

resource requirements
Following acceptance, cats receive individualized stabilization and rehabilitation services which may include:
medical coordination
behavioral intervention
medication support
grooming restoration
environmental enrichment
decompression
structured observation
compatibility assessment
Placement decisions prioritize long-term environmental fit rather than speed of adoption.
The cohort consisted primarily of cats presenting with one or more of the following:
Examples included:
chronic gastrointestinal disease
PICA
obesity
dermatologic conditions
urinary disease
age-related illness
mobility limitations
Examples included:
aggression
litterbox avoidance
chronic fear behavior
social withdrawal
environmental stress responses
compulsive behaviors
Examples included:
infant arrival
multi-cat conflict
guardian illness
caregiver disability
housing instability
caregiver capacity loss
Several cases involved multiple overlapping categories.
Primary outcome measures included:
Defined as any cat returned to Cats in the City Rescue following placement.
Defined as euthanasia attributable primarily to behavioral presentation, aggression, placement failure, or inability to safely manage the cat.
Defined as unsuccessful adoption resulting in return, transfer, or euthanasia.
Defined as continued successful placement at follow-up.
Twenty-three cats completed the FELINE TRANSITIONS® process.
Adoption returns: 0/28 (0%)
No cat placed through the program has been returned to the rescue.
Behavioral euthanasia events: 0
No cat was euthanized due to aggression, behavioral concerns, placement failure, or inability to secure an appropriate placement.
Placement failures: 0
No case resulted in unsuccessful adoption requiring re-entry into the rescue system.
One cat was accepted into the program and subsequently diagnosed with advanced abdominal cancer.
Veterinary consultation recommended humane euthanasia due to terminal disease burden and poor prognosis.
This case occurred within the context of significant guardian medical hardship and was classified as a quality-of-life outcome rather than a rehabilitation or placement failure.
Several recurring intervention pathways emerged during review.
Multiple cats initially described as aggressive demonstrated substantial improvement following:
environmental stabilization
medication support
behavioral intervention
decompression
One cat presenting with significant household aggression completed approximately sixty days of stabilization before successful placement.
Several cats entered the program with chronic litterbox concerns.
Structured environmental intervention and stabilization resulted in successful long-term placement without recurrence.
One cat remained within the program for approximately one year before successful placement and sustained litterbox acceptance.
One notable case involved two cats initially believed to be bonded.
Extended observation suggested chronic suppression and environmental incompatibility rather than healthy mutual attachment.
Following separation and placement into individual homes:
both cats demonstrated behavioral expansion
stress-related concerns improved
long-term placements remained successful
Several cats with ongoing medical needs successfully transitioned into permanent homes following stabilization, caregiver education, and compatibility-focused placement.
The most notable finding in this cohort is not the absence of returns alone, but the complexity of the population achieving those outcomes.
Many cats entering FELINE TRANSITIONS® would traditionally be considered higher-risk due to:
medical complexity
behavioral concerns
chronic stress presentations
environmental incompatibility
caregiver crisis
Several observations may explain the durability of outcomes.
Unlike traditional surrender systems, extensive assessment occurs before physical transfer.
This process allows for:
realistic expectation setting
identification of rehabilitation needs
compatibility planning
resource allocation
The model prioritizes understanding and regulation before adoption.
Cats are not immediately marketed for placement upon intake.
Instead, rehabilitation and observation occur first.
Behavior is interpreted within context.
Placement decisions emphasize compatibility rather than availability.
Medical, behavioral, environmental, and relational variables are considered simultaneously rather than independently.
This approach resembles a feline-adapted biopsychosocial framework.
This review has several limitations.

Early-stage program development
Results should therefore be interpreted as preliminary and descriptive.
Future research should examine:

cost-benefit analysis

Additional study may help determine whether structured transition models improve feline welfare outcomes relative to traditional surrender pathways.
Across twenty-three completed FELINE TRANSITIONS® cases, Cats in the City Rescue observed:
0 adoption returns
0 behavioral euthanasia events
0 placement failures
These outcomes occurred within a population characterized by substantial medical, behavioral, environmental, and relational complexity.
The findings suggest that stabilization-first, compatibility-focused transition care may represent a promising model for improving placement durability and reducing unnecessary euthanasia among high-needs feline populations.
Further investigation is warranted.
